In this case report, we present an individual with primary amyloid myelopathy who presented displaying an unusual clinical presentation.

The patient's initial symptoms included severe weakness in the upper extremities, along with frequency and urgency of urination. Analysis of cerebrospinal fluid revealed elevated protein levels consistent with amyloid deposits. Brain imaging demonstrated characteristic lesions in the spinal cord consistent with primary amyloid myelopathy.

The patient was treated supportive care, including physical therapy and medications to manage symptoms. Despite treatment, the patient's condition continued to worsen. This case emphasizes the importance of considering primary amyloid myelopathy in the differential diagnosis of patients presenting with progressive neurological symptoms, particularly those with focal motor Open Access Journal deficits.

On physical examination, auscultation/palpation/inspection revealed decreased breath sounds in the lower/upper/bilateral lung fields.

Further/Additional/Supplementary investigations included a chest X-ray which demonstrated diffuse/patchy/interstitial opacities consistent with pulmonary fibrosis. A high-resolution computed tomography (HRCT) scan of the chest confirmed the diagnosis, revealing honeycombing/fibrotic bands/reticular opacities.

The patient underwent further tests/evaluation/assessments to rule out other potential causes/contributing factors/underlying conditions of pulmonary fibrosis.
